Can Insomnia Make Muscle Pain Worse? 

From a rehabilitation Physician point of view, people living with chronic musculoskeletal pain frequently report difficulty sleeping, while those who sleep poorly consistently experience more pain. This is not a matter of coincidence; it reflects a well-established bidirectional relationship between sleep quality and pain perception, supported by decades of research in neuroscience and pain medicine.

Why Deep Sleep Matters for Muscle Recovery 

Quality sleep, particularly deep non-REM sleep (Stage 3), is when the body releases the majority of its daily “growth hormone”, the hormonal driver of tissue repair and muscle recovery. Without sufficient time in this restorative sleep stage, the microtrauma that muscles accumulate during everyday activities and exercise cannot be fully repaired. Over days and weeks of poor sleep, this creates a state of persistent low-grade tissue irritation that contributes to ongoing pain.

Why Lack of Sleep Can Make You Feel More Pain 

Sleep deprivation also has a direct and measurable effect on pain thresholds. Research demonstrates that sleeping less than six hours per night for just a few consecutive days produces central sensitization, a state in which the central nervous system becomes hyperresponsive to pain signals. Stimuli that would normally register as mild discomfort are perceived as significant pain. This neurological amplification explains why everything aches after a bad night’s sleep, and why chronic pain patients often report their worst days following poor sleep.

At the biochemical level, sleep deprivation elevates inflammatory markers including IL-6 and TNF-α. For individuals with existing musculoskeletal conditions; osteoarthritis, rheumatoid arthritis, fibromyalgia, or tendinopathy, this rise in systemic inflammation further aggravates local tissue irritation. Many patients with these conditions describe their symptoms as directly tracking sleep quality from the previous night.

The resulting cycle is familiar to many chronic pain sufferers: pain makes it hard to fall or stay asleep → poor sleep amplifies pain sensitivity → waking with worse pain → increased anxiety and distress → even greater difficulty sleeping. Breaking this cycle requires simultaneous attention to both the pain and the sleep problem, addressing only one side is rarely sufficient for lasting improvement.

Evidence-based strategies for improving sleep in pain patients include 

  • maintaining consistent sleep and wake times seven days a week 
  • eliminating blue-light screen exposure for at least one hour before bed
  • engaging in gentle evening movement (stretching, yoga, or walking) to reduce muscle tension
  • and most importantly; Cognitive Behavioral Therapy for Insomnia (CBT-I), which is the most effective long-term treatment for chronic insomnia and is recommended over sleep medications by most clinical guidelines.

From a physical therapy perspective, a structured pre-sleep stretching and relaxation routine can significantly reduce accumulated daytime muscle tension and prepare the body for deeper, more restorative sleep. Physiotherapists can design individualized evening movement programs targeting the specific areas where each patient holds their tension, whether that is the neck, hips, lower back, or shoulders.

The Cycle of Chronic Pain and Poor Sleep

Restorative sleep is not a luxury, it is a fundamental pillar of musculoskeletal health.
