Many patients ask, “Why is the pain still not fully gone after all this physical therapy?” This article explains the possible reasons and the right approach for treatment that lasts.

1. An Inaccurate Diagnosis

The most important element of treatment is an accurate diagnosis. If the true source of the pain is not correctly identified, treatment becomes nothing more than patching the symptom. For example, upper back pain that looks like inflamed muscles may actually be caused by a herniated disc compressing a nerve, or even by a problem in the spinal facet joints themselves. If the diagnosis is wrong, physical therapy focused only on muscle relaxation will not fix the problem at its root.

2. Lack of Continuity and Treatment Plan Adjustment

Physical therapy is not a one-and-done treatment — it requires continuity and discipline. Attending every scheduled physical therapy session is essential. In addition, the treatment plan should be regularly “adjusted” to match the patient’s symptoms and progress. If the current plan isn’t producing results, it needs to be reassessed and revised, or other adjunctive treatments may be considered.

3. Daily Habits That Are Still Unhealthy

Even with regular physical therapy, if the patient continues with “risky behaviors” that repeatedly trigger the pain, treatment won’t be as effective as it should be. Examples include sitting with poor posture while working, lifting heavy objects incorrectly, using a computer for long hours without breaks, or even sleeping in unsuitable positions. Changing these habits is just as important as the physical therapy itself.

4. Insufficient Muscle Strength

Many types of chronic pain, especially in the back and neck, are often caused by weak muscles and an inadequate core. These muscles support the body’s structure and help absorb shock. When they are weak, the joints and tendons have to work harder, leading to pain. Physical therapy therefore usually includes strengthening these muscles — but patients need time and consistent, sustained effort to see good results.

5. Psychological Factors

Stress, anxiety, or depression can affect how pain is perceived, making it feel more intense and slower to resolve. These psychological factors are often overlooked, yet they play an important role in managing chronic pain. Caring for mental health alongside physical treatment is essential.

6. Treatment Takes Time

Chronic pain usually results from problems that have accumulated over a long period, so treating it properly requires time and patience — results cannot be seen overnight. Patients should understand that the recovery process takes time and discipline in closely following the advice of the physician and physical therapist.

7. The Root Cause Is Degeneration of the Body

Degeneration of the body, especially in older adults, produces chronic pain. Joint degeneration, low muscle mass (sarcopenia), and a slower rate of physical recovery than in younger people are all factors that keep chronic pain from resolving completely.

If you are dealing with chronic pain that just won’t let up, I recommend seeing a rehabilitation physician at Freesia Rehab Clinic to identify the cause and build a comprehensive treatment plan covering physical therapy, habit change, and holistic health care — so you can get back to living happily and manage your pain sustainably.

Still in pain after many physical therapy sessions?
