What to Know Before Starting Treatment

This is, without question, the most common concern I hear from new patients. I genuinely welcome the question because understanding what to expect makes you a better participant in your own recovery, and a more communicative patient leads to better outcomes.

The most accurate answer is: some mild discomfort during certain techniques is completely normal, but physical therapy should never be sharply painful or intolerable. The distinction between therapeutic discomfort and warning-sign pain is critical, and it is something you should communicate openly throughout every session.

Sensations You May Experience During or After Physical Therapy

Muscle fatigue or muscle soreness after session, delayed onset muscle soreness (DOMS), the familiar ache 24–48 hours after exercise-based therapy, is a normal physiological response, identical to starting a new workout. Mild pressure or a sensation of “stretching” during joint mobilization or manual therapy is also expected and typically resolves within moments.

When Should You Tell Your Physiotherapist Immediately?

sharp, shooting, or electric pain during treatment; numbness or tingling that worsens during or after a session; significant increase in pain lasting more than 48 hours post-treatment. These signals indicate that the treatment intensity or approach needs adjustment, and a good therapist will respond immediately when you speak up.

What Does Each Type of Physical Therapy Treatment Feel Like?

Therapeutic ultrasound creates a gentle warmth. TENS (Transcutaneous Electrical Nerve Stimulation) produces a mild buzzing or tingling. Shockwave therapy feels like a rhythmic deep dull tap, often described as slightly uncomfortable in the first session but significantly more tolerable in subsequent ones as the tissue responds. A high power laser creates pleasant deep warmth. Joint mobilization may produce a brief “stiffness releasing” sensation.

An important misconception to correct effective physical therapy is not measured by how much it hurts. Pain is not a proxy for therapeutic benefit. Our physiotherapists at Freesia Rehab Clinic work within a carefully calibrated zone of therapeutic challenge, enough to drive tissue adaptation and strength gains, but never enough to provoke guarding, fear, or injury.

How to Prepare Before Your First Physical Therapy Session

Having a meal not too full, note your current pain level on a 0–10 scale and when it is best and worst; bring a list of current medications; inform your therapist of any previous surgeries or treatments; and most importantly, speak up immediately if anything feels wrong. There are no prizes for tolerating unnecessary pain.

Start Physical Therapy with Confidence at Freesia Rehab Clinic

At Freesia Rehab Clinic, we will walk you through every step of the assessment and explain what each technique involves before we begin. Recovery built on trust and open communication is always more effective than one built on uncertainty.

References:

1) APTA. “What to Expect in Physical Therapy.” choosept.com.
2) Spire Healthcare. spirehealthcare.com.
3) Freesia Rehab Clinic. freesiarehabclinic.com

This is, without question, the most common concern I hear from new patients.
